How Do I Become a New Patient?

Pediatric Services 360 for your child’s care. Whether you’re welcoming a newborn or transferring from another pediatrician, our goal is to make the process smooth, supportive, and clear.
For Newborns
Welcoming a new baby is a joyful and sometimes overwhelming time. Beginning the registration process toward the end of your second trimester, helps ensure a smooth transition of care once your baby arrives. Establishing care early allows your family to build trust and consistency with our team right from the start.
For Families Transferring to Us
If you’re transferring from another pediatrician, we’ll work with you to gather your child’s complete medical history—including immunizations, growth charts, and past test results. This step ensures continuity of care and helps us provide the highest standard of pediatric care for your child.

Important Reminders
- Completing a New Patient Application does not mean we have accepted medical responsibility for your child. All applications must go through an acceptance review by our administration.
- We do everything possible to have your child’s medical records before their first visit. These records give us the health history we need to provide safe, consistent care from day one.
- Federal (HIPAA) and state laws allow your previous provider to charge a
reasonable, cost-based fee for copying records. This cost is the family’s responsibility, though fees are often waived when records are sent directly to our office.
Our Process for New Patient Application
Here's how it works:
1. New Patient Interest Form: Call our office to complete a form with our front desk team. This helps us learn more about your child and begin the review process.
2. Review & Approval: All applications are carefully reviewed by our team to ensure we can meet your child’s needs. Please allow 1-2 business days for this step. Completing the form does not guarantee acceptance into our practice.
3. Pick Up Your Packet: When approved, you’ll be invited to obtain your New Patient Packet, which includes consent forms for requesting records from your previous provider.
4. Records Transfer: Once we’ve received and reviewed your child’s complete medical records. However, if your child has a time-sensitive medical need, we will make every effort to accommodate the visit and perform the necessary examination while records are pending.
