What to Bring to Your Baby’s First Appointment
When you find out you’re expecting, give our office a call. Our front desk team will help you complete a simple pre-registration form over the phone, which will then be reviewed by our administration.
Once approved, you’ll be invited to collect your New Patient Packet and any consent paperwork. Please bring these completed forms with you to your baby’s first appointment.
After delivery, you can let the hospital staff know that you’ve chosen Pediatric Services 360 as your pediatrician.
